How to settle the dissatisfaction of patients against the Emergency room.

응급실 내원객들이 느끼는 불만양상

Abstract
Background
: In these days of increasing need for medical treatment, and the trend of changing lifestyles for personal health care, the position of medical treatment has led to the increase of careful and various services to every member of the community. But the reality of medical treatment, including the present situation of the emergency room in general hospital is recognized as the cause of those dissatisfactions and complaints of both patients and their caregivers. So this study aims at contributing to the harmonious relation between doctor and patient.

Methods : This study has been carried out by the questionnaire method on the basis of the relatively reasonable 331 papers among 430 questionnaire papers of the patients and their care givers hospitalized by way of the emergency room in Cheonju Yeong Dong Hospital from Sep. 1994 to Apr. 1995.

Results : Among 331 subjects of this questionnaire, there were 121 male patients(36.6%), 76 female patients(23.0%), 80 male caregivers(24.2%), 54 female caregivers. The subjects were compaced of 23 teenagers(7.8%), 74 in their twenties(22.4%), 135 in their thirties(40.8%), 52in their fourties(15.7%), 29 in their fifties(8.8%), 12 in their sixties(3.6%), 6 in their seventies(1.8%). Both patients and their caregivers seemed to have a great dissatisfactions and complaints, which can be extraneous to medical treatment. Many patients complained of the unbearable situations such as delayed pain control before after a first-aid treatment, as well as various poor condition of medical treatment in emergency room. Their caregives complained of the lack of a thought explanation of diagnosis and prognosis, as well as the tedious waiting time before taking medical teratment. There were many cases where the emergent patients took medical treatment with some dissatisfactions and complaints of medical staffs or medical donditions.

Conclusion : When using the emergency room in a general hospital, both patients and their caregivers experiance a great deal of dissatisfactions and complaints, which can be followed by a distrust of the hospital and an obstacle of medical treatment. The medical staff, such as the hospital authorities, doctors, and nurses should make various improvements upon the medical conditions of the emergency room in general hospital.

연구배경 : 증가되는 의료의 욕구와 건강을 지키기 위한 생활양식의 변화가 많은 지금, 의료의 위상은 사회구성원 모두에게 다양하고도 세심한 서비스로서의 의료로 유도되고 있다. 그러나 의료현실, 특히 종합병원 응급실에서의 상황은 아직도 많은 불만과 갈들이 내재되어 있다. 많은 수의 환자, 보호자들은 응급실을 이용하면서 진료, 진료외적으로 부딪히는 여러 문제로 불만을 토로하는 바 저자등은 이러한 불만이 생기는 원인과 배경을 살펴 문제점을 올바로 인식하고 이의 해소방안을 고려함으로써 원만한 의사-환자 관계형성에 도움이 되고자 하였다.

방법 : 1994년 9월부터 1995년 4월 사이에 병원 응급실을 경유해 입원하게 된 환자와 그 보호자들에게 설문 조사를 이용, 분석적 단면 연구를 시행하였다. 회수된 430편의 설문서중 본 연구에 비교적 타당한 내동들로 된 331편의 설문서를 기초자료로 해서 분석하였다.

결과 : 설문응답자 331명중 남자 환자는 121명(36.6%), 여자 환자는 76명(23.0%), 남자 보호자는 80명(24.2%), 여자 보호자는 54명(16.3%)이었다. 또 응답자의 연령별 분포는 10대 23명 (7.8%), 20대 74명(22.4%), 30대 135명(40.8%), 40대 52명(15.7%), 50대 29명(8.8%), 60대 12명(3.6%), 70대 6명(1.8%)이었다. 환자의 보호자들은 본 병원에서 직접 경험했었던 불만들이 많았으며 진료외적 불만들도 상당수 있었다. 환자군에 있어서는 응급처치를 전후해서 첫째, 응급실에서 환자의 힘든 상황(통증 등)을 빠른 시간에 해결해 주지않은 점과 둘째, 응급실의 제반 치료환경에 대한 불만족 등의 순으로 불만이 나타났고 보호자 군에서는 첫째, 진료를 받기 전의 긴 대기시간, 진단과 예후 대한 상세한 설명부족의 순으로 부만을 나타내었다. 응급환자들은 병원 응급실에서 여러 가지 불만들을 갖고 있었으며 진료를 받을 때 이미 불만을 지니고 진찰, 혹은 처치를 받는 경우 많이 있었다.

결론 : 환자나 보호자들이 병원응급실을 이용하는 데에서 여러 가지 불만들이 있었으며, 이러한 불만들은 병원에 대한 불신으로 이어질 수 있고 나아가 치료의 장애요인이 될 수도 있을 것이다. 병원당국이나 의사, 간호자 등 제반치료의 주체가 되는 인적, 물적 요소에서 이러한 불만들이 어떤 문제로 인해 발생하는가를 잘 파악하고 분석을 해서 병원응급실의 제반환경을 개선해 나아가야 할 것이다.
