| Abstract |
Background
: Inspite of the significance of early detection for glaucoma, the majrity of primary care physician does not mesure intraocular pressure in their setting due to limitation of technique for conventional tonometry. The purpose of this study was to compare the effectiveness of noncontact tonometry(NCT) using the air-puff noncontact tonometer(CT-50, Topcon, Ltd, Japan) with that of conventional tonometry using the Goldmann applanation tonometer as a screening tool for glaucoma.
Methods : Intraocular pressure was measured by a technician using the air-puff NCT 3 times and by a skillful ophthalmologist using the Goldmann applanation tonometry once in both eyes of 153 patients who visited eye clinic from June, 1993 to July. 1993.
Results : Air-puff NCT correctly identified 12 of the 13 eyes with an intraocular pressure above 21mmHg measured by Goldmann applanation tonometry for a sensitivity of 92.3%, and 267 of the 268 eyes with an intraocular pressure less than 21mmHg for a specificity of 91.1%. The two methods of intraocular pressure measurement yielded similar pressure readings. The correlation coefficinet is 0.91(Y=0.80X +1.81).
Conclusion : Air-puff NCT is an easy, safe, and practical method of intraocular pressure measurement. So we suppose that air-puff NCT can availably be used in primary care setting to detect glaucoma. |

연구배경 : 녹내장에 있어서 조기발견이 중요하다는 것을 인식하면서도 기존의 안압계가 갖고 있는 기술적인 제한점 때문에 대부분의 일차의료인들이 일차진료 현장에서 안압측정을 하지 않고 있는 실정이다. 본 논문의 목적은 Air-puff 비접촉성 안압계(CT-50, Topcon, Ltd, Japan)의 녹내장 검색도구로써의 유용성을 기존의 Goldmann압평 안압계와 비교하고자 하였다.
방법 : 1993년 6월부터 7월사이에 명인안과를 방문한 153명의 환자(306안)를 대상으로 안압의 측정은 먼저 의사가 아닌 한명의 기사가 Air-puff 비접촉성 안압계로 3회 측정한 다음 숙련된 안과의사가 Goldmann압평 안압계로 1회 측정하였다.
결과 : Air-puff 비접촉성 안압계의 측정에서 Goldmann 압평 안압계에 의해 21mmHg를 넘는 안압을 보인 13안중 12안을 정확히 판별하여 민감도 92.3%를 보였으며, 21mmHg 이하인 293안중 267안을 정확히 판별하여 91.1%의 특이도를 나타냈다. 두가지 안압 측정방법에서 유사한 측정치를 보였으며 상관계수는 0.91(Y=0.80X +1.81)이었다.
결론 : Air-puff 비접촉성 안압계는 간편하고 안전하며 실용성있는 안압 측정도구로 일차진료 현장에서 녹내장의 검색도구로 이용하는데 손색이 없을 것으로 생각된다. |
