| Abstract |
Background
: This study was attempted to evaluate the influence of the family function on individual health by analyzing an association between Smilkstein's APGAR Score and depression scales, anxiety scales, degree of fatigue.
Methods : Data were gathered by questionnaires which is consisted of Smilkstein's APGAR Score, Beck's depression inventory(BDI), Zung's self-rating anxiety scale(SAS), and subjective complaints of fatigue from 239 passers of Jeonju Teachers college in 1992.
Results : The mean of Family APGAR Score was 13.74±3.33 and among them the dysfunctional group(point 0 to 13) was 113(47.3%), and functional group(point 14 to 20) was 126(52.7%). There was significant difference between dysfunctional and functional group in depression and anxiety score(P<0.05), but no significant difference in degree of fatigue(P>0.05). The depression scores of each APGAR item showed a significant difference between satisfactional and dissatisfactional groups in all items(P<0.05). Also, there were meaningful difference in all anxiety scores except affection item as well as in all fatigue scores except adaptation item between two groups.
Conclusion : These results suggested the family APGAR Score has an association with depression, anxiety, fatigue of individual family members. Thus, it is recommanded to approach for evaluation of family function in treatment of depressive, anxious, or fatigued patients. |

연구배경 : Smilkstein의 가족 기능도 지수와 우울, 불안 점수 및 피로도의 관계를 분석해 봄으로써 가족 기능이 개인의 건강에 미치는 영향을 알아보고자 한다.
방법 : 1992년 전주 교대 1차 합격자 268명을 대상으로 Smilkstein의 가족 기능도 지수, Beck씨 우울 척도(BDI), Zung씨 자가 불안 척도, 피로 자각 증상표(일본산업위생협회)를 이용해 자기보고식 설문조사하여 수면 시간이 5시간 이하이거나 과거 혹은 현재에 질병을 가지고 있거나 약물 복용중인 29명을 제외한 239명을 대상으로 SPSS로 전산처리해 T-test와 chi-square test로 분석하였다.
결과 : 조사 대상자의 성별은 남자가 61명(25.5%), 여자가 178명(74.5%)이었으며 연령은 16세에서 25세 사이로 평균 18세였다. 20점을 총점으로 해서 본 가족 기능도 지수의 평균치는 13.74±3.33이었으며, 기능 장애군(dysfunctional group)이 113명(47.3%), 기능군(functional group)이 126명(52.7%)이었다. 가족 기능도 지수의 기능 정도와 우울 및 불안 점수와는 우울점수가 기능 장애군에서는 10.08±4.33 기능군에서는 7.45±4.07, 불안 점수가 기능 장애군에서는 35.00±4.39 기능군에서는 33.00±6.09로 상호 유의한 관계가 있었다(P<0.05). APGAR 항목별 우울 점수는 A, P, G, A, R 5항목 모두에서 만족군과 불만족군 사이에 유의한 차이가 있었고(P<0.05) 불안 점수는 애정도(Affection) 항목을 제외한 4항목에서 만족군과 불만족군 사이에 유의한 차이가 있었다(P<0.05). 가족 기능도 지수의 기능 정도에 따른 피로도의 비교에서는 상호 관계가 없었다(P<0.05). 각 항목별 피로도는 적응도(Adaptation)를 제외한 4항목에서 만족군과 불만족군 사이에 유의한 차이를 보였다(P<0.05).
결론 : 이상의 결과로 우울, 불안, 피로 등을 보이는 환자 진료에서 가족 기능 평가 및 가족 기능 강화를 위한 접근이 필요하리라 생각된다. |
