Abstract

Methods : Coping scores and APGAR scores were taken 137 of patients and their families who were admitted to hospital between March and May in 1994. Statistical reliability testing was performed with Cronbach's alpha in SPSS/PC+ 4.0 and a comparison was made between the total coping score and individual items scores to find any correlation. Two technique were incorporated to determine the validity of the scoring, first, interviews were performed on families with chronic illness and points given from 1 to 5 for families with increasing levels of coping to the illness, and these results were compared with the coping score for correlation, second, APGAR scores were compared with the coping score for correlation. SAS was need to determine correlation.

초록

연구배경 : 뇌졸중과 같이 만성적인 장애를 초래하는 질병은 개인뿐만 아니라 그 가족에게도 심각한 영향을 끼친다. 만성질환자를 돌보는 가족의 기능을 평가하여 환자와 환자의 가족이 질병에 효과적으로 대처해 나가도록 도울 필요성이 있다.

방법 : 뇌졸중환자가족을 실험군으로 고혈압환자가족을 대조군으로 하여 가족기능평가 도구인 Family APGAR와 FACESⅢ로 설문조사하고, 뇌졸중환자의 후유장애는 Barthel Index로 평가하였다.

결과 : 1. Family APGAR score는 뇌졸증군이 7.38±2.62, 고혈압군이 6.85±2.64였으나 통계적으로 유의하지는 않았고, 항목별로는 성장도가 뇌졸중군 1.57±0.65, 고혈압군 1.32±0.67로 뇌졸중에서 유의하게 높았으나 가족유형의 차이는 없었다.
2. 가족결속력은 뇌졸중군에서는 이탈군이 28가족(41.2%), 연결군 20가족(29.4%), 분리군 12가족(17.6%), 밀착군 8가족(11.8%)였고 고혈압군에서는 이탈군 36가족(53.0%), 분리군 19가족(28.0%), 연결군 10가족(14.8%), 밀착군 3가족(4.4%)의 순이었으며 통계적으로 유의하였다.(P<0.05)
3. 가족적응력과 가족구조유형은 양군간에 의미있는 차이가 없었다.
4. 뇌졸중환자의 장애정도(Barthel Index)와 Family APGAR score, 가족결속력, 가족적응력, 가족구조유형사이에는 우의한 차이가 없었다.
5. Barthel Index와 APGAR score사이의 상관계수는 0.254(p<0.05)로 약한 상관관계가 있었다.
6. 뇌졸중의 유병기간과 Family APGAR, 가족결속력, 가족적응력, 가족구조유형사이에는 유의한 차이가 없었다.

결론 : 뇌졸중환자를 돌보는 것이 가족의 기능을 나쁘게 만들기 보다는 가족구성원간의 결속력을 강화시키고, 성장시키는 쪽으로 작용하였다.
