Comparison of Fatigue Severity and Quality of Life between Unexplained Fatigue Patients and Explained Fatigue Patients

Background
Recently, despite the high prevalence of fatigue in patients, there is a lack of research on the quality of life (QoL) in unexplained fatigue patients, indicating that they are not properly diagnosed and treated. The aim of this study was to compare fatigue severity and QoL between patients with explained and unexplained fatigue.
Methods
The study consisted of 200 Korean adults who complained of fatigue without underlying disease. Fatigue Severity Scale, Short Form Health Survey-36 version 2 (SF-36v2), and Beck Depression Inventory-II (BDI-II) selfquestionnaires were administered. Participants were dichotomized to two groups, namely, patients with unexplained or explained fatigue, sorted according to laboratory examination results. The chi-square test, t-test, and Wilcoxon rank-sum test were used, and analysis of covariance was calculated after adjusting for age, sex, body mass index, smoking status, and physical component summary (PCS) of SF-36v2 or BDI-II.
Results
PCS of SF-36v2 between the two groups showed significant difference. Compared to patients with explained fatigue, those with unexplained fatigue showed lower physical component scores of QoL.
Conclusion
QoL of patients with unexplained fatigue could largely diminish than those with explained fatigue. The primary clinician should be aware of poor QoL in patients with unexplained fatigue to identify who is in need of more attention and intervention.

Association between Leukocyte Mitochondrial DNA Copy Number and Regular Exercise in Postmenopausal Women

Background

Previous studies suggest that habitual exercise can improve skeletal mitochondrial function; however, to date, the association between exercise and mitochondrial function in peripheral leukocytes has not been reported. The aim of this study was to evaluate the relationship between regular exercise and mitochondrial function by measuring leukocyte mitochondrial DNA (mtDNA) copy number in postmenopausal women.

Methods

This cross-sectional study included 144 relatively healthy, non-diabetic, non-smoking, postmenopausal women. Clinical parameters, including anthropometric measurements and cardio-metabolic parameters, were assessed. Regular exercise was defined as at least 150 minutes per week of moderate-intensity activity, or an equivalent combination of moderate and vigorous-intensity activity, over a duration of at least 6 months. Leukocyte mtDNA copy numbers were measured using real-time polymerase chain reaction assays, and these were normalized to the β-globin copy number to give the relative mtDNA copy number.

Results

The mtDNA copy number of peripheral leukocytes was significantly greater in the exercise group (1.33±0.02) than in the no exercise group (1.05±0.02, P<0.01). Stepwise multiple regression analysis showed that regular exercise was independently associated with mtDNA copy number (β=0.25, P<0.01) after adjusting for the variables age, body mass index, waist-to-hip ratio, systolic and diastolic blood pressure, homeostasis model assessment of insulin resistance value, and levels of high-density lipoprotein cholesterol, triglycerides, and homocysteine.

Conclusion

Regular exercise is associated with greater leukocyte mtDNA copy number in postmenopausal women.

Triage of Non-urgent Ambulatory Patients by Family Medicine Resident in Emergency Room.

Background
This study was performed to evaluate the validity of triage of non-urgent ambulatory patients using Triage Tool, the Emergency Severity Index-4 by a family medicine resident in the emergency room (ER). Methods: A total of 790 ambulatory patients who visited an urban ER between March and April 2007 were enrolled. A family medicine (FM) resident and emergency medicine (EM) residents independently evaluated the severity of patients with ESI-4, and reviewed the basic characteristics and disposition of the patients. Concurrent validity and predictive validity were measured using weighted kappa analysis and chi-square analysis. Results: Concurrent validity was good. The weighted kappa value was 0.910 between an FM resident and EM residents. The hospitalization rate was 75% in catergory 2 and 19.6%, 2.5%, 0.05% in category 3 through 5, respectively. More severe category patients of ESI-4 had higher rate of hospitalization rate. Conclusion: Triage by a family medicine resident, using the Emergency Severity Index-4 in ambulatory patients of emergency room (ER) is a useful screening tool for non-urgent patients. Value of the Vital Sign in Hypoglycemia Presenting Mental Change on Prehospital Management.

Background
The clinical guideline for prehospital blood glucose measurement in hypoglycemia presenting with mental change has not been sure, maybe it has depended on individual decision. Therefore we was going to find out whether the difference of the vital sign between hypoglycemia and non hypoglycemia can give important message or not. Methods: Retrospective study was carried out from Jan 2004 to Dec 2005. The patients with mental change were divided into hypoglycemic group and non hypoglycemic group. Then we compared the difference of vital sign (Blood pressure, respiration rate, heart rate) based on paramedic run reports and medical records. Results: 66 person in hypoglycemic group were aged 60.92⁑16.90 on average. And 67 person in non hypoglycemia were 58.53⁑16.58. The difference of blood pressure, respiration rate is not significantly but only body temperature makes significant difference(P=0.014). Furthermore it was inclined that the lower body temperature was more likely in hypoglycemia(P<0.001). Compared with over 36.0oC, probability of hypoglycemia in less than or equal to 36.0oC of temperature was more (OR: 54.28, 95% CI: 18.956∼155.464). Conclusion: The body temperature gives more significant information in Prehospital blood glucose measurements for hypoglycemic patients with mental change is not absolutely, but recommended in less than 36.0oC. The Relationship between Bone Mineral Density and Body Composition Variables Measured by DEXA in Postmenopausal Women.

Background
: This study was aimed to quantify the relationship between the bone mineral density and the body composition in Korean postmenopausal women.

Methods : Whole and regional body composition and bone mineral density (BMD) were measured with DEXA in 149 postmenopausal women. Data were analysed using ANOVA test, Pearson correlation and multiple linear regression.

Results : The mean age was 53.4±6.9 years, the mean years since menopause(YSM) was 5.4±6.7 years, the mean height was 154.5±4.9 cm and the mean weight was 56.8±7.4 kg. They were classified under non-osteoporosis (n=40), osteopenia (n=66) or osteoporosis (n=43) group. There were significant differences in age, YSM, height, total lean body mass, bone mineral content (BMC), regional lean body mass of leg and trunk and regional BMD among the three groups (P<0.05). In contrast, weight, BMI, total fat mass and body fat percentage were not significantly different among the three groups. The BMD of total body, lumbar spine and proximal femur had a negative correlation with age and YSM but positive correlation with height, weight, total lean body mass (P<0.05). From multiple linear models, age, total lean body mass, total fat mass and YSM were found to be the best predictor of total BMD (R2=0.460, P<0.05) and age and total lean body mass were found to be the best predictor of BMD of lumbar spine (R2=0.223, P<0.001) and proximal femur (R2=0.351, P<0.001).

Conclusion : From these results it is suggested that lean body mass is the major determinant of BMD at total, lumbar spine and proximal femur in postmenopausal women.
