Case Report

A Case of Acute Pancreatitis: An Unusual Manifestation of Acute Q Fever

Acute pancreatitis is a sudden inflammation affecting the exocrine region of the pancreatic parenchyma. Infectious etiologies are rare. Here we report an exceptional case of a 44-year-old woman from a rural area who was referred to our hospital with fever and abdominal pain. A physical examination revealed pale skin and epigastric tenderness. Thoracoabdominal computed tomography revealed a Balthazar score of D. Serum laboratory findings revealed hemolytic anemia, hepatic cytolysis, and high C-reactive protein level. Calcium and lipase levels were normal. There was no history of recent trauma, alcohol consumption, or drug intoxication. The diagnosis of “query” pancreatitis was confirmed by serological Coxiella burnetii positivity. Oral doxycycline 200 mg daily was initiated. The clinical evolution was favorable. To our knowledge, no association between acute pancreatitis and hemolytic anemia caused by C. burnetii was reported previously. Q fever must be considered in cases of acute pancreatitis, especially when the patient is from a rural area or has a high-risk profession.

Mild Anemia and Risk for All-Cause, Cardiovascular and Cancer Deaths in Apparently Healthy Elderly Koreans

Background
Being common, mild anemia is sometimes considered a mere consequence of aging; however, aging alone is unlikely to lead to anemia. Therefore, this study aimed to investigate the association between mild anemia and total mortality and cause-specific mortality in apparently healthy elderly subjects.
Methods
A retrospective cohort study was conducted on 10,114 apparently healthy elderly individuals who underwent cancer screening and routine medical check-ups at one Health Promotion Center between May 1995 and December 2007. We defined mild anemia as a hemoglobin concentration between 10.0 g/dL and 11.9 g/dL in women and between 10.0 g/dL and 12.9 g/dL in men. We assessed the relationship between the overall, cardiovascular (CV), and cancer mortality and mild anemia using Cox proportional hazard models.
Results
Mild anemia was present in 143 men (3.1%) and 246 women (6.1%). During an average follow-up of 7.6 years, 495 deaths occurred, including 121 CV and 225 cancer deaths. After adjustments, mild anemia was associated with a 128% increase in the risk of all-cause mortality hazard ratio (HR, 2.28; 95% confidence interval [CI], 1.54– 3.37) in men and cancer-related mortality (HR, 2.25; 95% CI, 1.22–4.13), particularly lung cancer (HR, 2.70; 95% CI, 1.03–7.08) in men, but not in women. In the subgroup analyses based on smoking status, obesity, and age, the associations were more prominent in never or former smoker groups and the older group.
Conclusion
The present study shows that overall and cancer-related mortality was associated with mild anemia in elderly men. Future prospective studies are needed to consolidate our findings.

BCL11A rs1427407 Genotypes in Sickle Cell Anemia Patients Undergo to Stroke Problems in Sudan

Background
Sickle cell disease is an autosomal recessive condition that results from the presence of a mutated form of hemoglobin. Some genetic variants of BCL11A are amenable to therapeutic manipulation. The present study investigated the relationship of a BCL11A variant (rs1427407) and its plasma levels with vaso-occlusive crises and stroke complications among patients in Sudan with sickle cell disease.
Methods
This cross-sectional study was performed between June 2014 and October 2016. The subjects included 166 patients who were diagnosed with sickle cell disease and 35 healthy control subjects, who were grouped according to sex and age (<15 years, 15–25 years, and >25 years). All patients and/or their guardians provided informed consent. Blood samples were collected from the patients and controls under aseptic conditions.
Results
Plasma BCL11A levels were elevated in cases with vaso-occlusive crises that lasted for >3 years. In addition, plasma BCL11A levels were high in cases with the GG genotype (vs. GT and TT) at rs1427407. Furthermore, the BCL11A rs1427407 GG/GT genotypes increased the risk of vaso-occlusive crisis and stroke in the patients with sickle cell disease.
Conclusion
The BCL11A variant (rs1427407) and its plasma levels were associated with vaso-occlusive crisis and stroke in patients with sickle cell disease.

The Effect of Education Based on the Theory of Planned Behavior on Iron Supplementation among Pregnant Women

Background
Iron is an essential element for women of reproductive age, especially in the period before and during the pregnancy. This study investigates the consumption of iron to prevent iron deficiency anemia among pregnant women visiting Neyshabur healthcare centers based on the theory of planned behavior.
Methods
In this experimental study, data were collected through a questionnaire survey. It included 160 pregnant women who were receiving maternity services at twelve healthcare centers in the city of Neyshabur in Iran between 2015 and 2017. The participants’ demographic and anthropometric characteristics, Using the theory of planned behavior, and blood lab examination results, including ferritin levels were measured and the data were analyzed using IBM SPSS ver. 22.0 (IBM Corp., Armonk, NY, USA).
Results
The average scores of knowledge, attitude, perceived behavioral control, subjective norms, and intention categories for the intervention group were meaningfully increased after the participants received education on iron supplementation (P<0.05). However, these changes were not found to be significant in the control group (P>0.05). No statistically significant difference was obtained in the subjective norms category between the two groups after the education intervention (P=0.92).
Conclusion
Based on the experimental effects of education encouraging iron supplementation in pregnant women, it is suggested that workshops promoting iron supplementation should be conducted in health centers with the aim of preventing widespread iron deficiency anemia.

Background

Although chronic obstructive pulmonary disease is a known cause of secondary polycythemia with elevated erythropoietic demands in response to hypoxemia, anemia is relatively common in patients with chronic obstructive pulmonary disease and is related to increased mortality. However, little is currently known about the relationship between various iron parameters and disease severity in chronic obstructive pulmonary disease patients.

Methods

Data from the fifth Korean National Health and Nutrition Examination Survey, a population-based epidemiologic survey conducted in 2010–2012, were used. A total of 1,129 patients with chronic obstructive pulmonary disease were examined to reveal the associations between the forced expiratory volume in 1 second (FEV1) and hemoglobin and iron parameters, including serum iron, ferritin, total iron binding capacity, and transferrin saturation, using Spearman correlations and multiple linear regression analyses.

Results

The FEV1 was positively correlated with serum hemoglobin (γ=0.37, P<0.001), iron (γ=0.20, P<0.001), transferrin saturation (γ=0.19, P<0.001), and ferritin (γ=0.22, P<0.001), and negatively correlated with age (γ=−0.40, P<0.001) and lower in female patients (γ=−0.56, P<0.001) in the Spearman correlation. The FEV1 was independently associated with serum iron (β=0.049, P=0.035) and transferrin saturation (β=0.049, P=0.035) after adjusting for age, sex, and body mass index in the multiple linear regression analyses.

Conclusion

The serum iron and transferrin saturation levels were independently associated with FEV1 as a marker of chronic obstructive pulmonary disease severity.

Case Report

Iron Deficiency Anemia due to Long-time Bloodletting Using Cupping.

We report three cases of iron defi ciency anemia due to long-time bloodletting using cupping. Case 1 was a 52-year-old man who sought evaluation at the Health Promotion Center in our hospital due to fatigue and dyspnea on exertion (DOE). There were no abnormal fi ndings on his general health examination, except anemia. He has performed self-bloodletting for a long time with cupping on his back and extremities for fatigue, myalgias, or polyarthralgias. Case 2 was a 52-year-old woman with fatigue and DOE. The physical examination revealed a systolic murmur at her left lower anterior chest and pale conjunctiva. The initial hematocrit was 22.4% and the hemoglobin was 6.4 g/dL. She has self-bloodletted using cupping 2 to 3 times a week when she felt tired or had myalgias. Case 3 was a 35-year-old man with sudden onset fatigue and DOE. He had severe DOE during a challenging physical test. He frequently received bloodletting using cuppings on his back and extremities by a doctor of Oriental medicine. There were no abnormal findings on the general health examination, except anemia. All three patients were diagnosed with iron defi ciency anemia due to chronic blood loss. We recommended stopping bloodletting using cupping and prescribed oral iron supplements.

The Association of Helicobacter pylori Infection and Anemia in Patients with Gastritis.

Background
: Helicobacter pylori has been known to be not only associated with gastrointestinal diseases but also to be reported in several cases of increased incidence of anemia. The anemia associated with H. pylori infection was reported though the population studies and case reports. But in Korea, there have been not enough studies especially in adults group, so we investigated to find out if H. pylori infection is associated with anemia in adults with gastritis.

Methods : Subjects were 382 men older than 40 years and 158 postmenopausal women, who had visited a general hospital for physical examination from May 2001 to October 2001 and diagnosed as only gastritis. Data were analysed for the effects of H. pylori infection on anemia using logistic regression test.

Results : In men, Pearson correlation coefficients between age and hemoglobin, and hematocrit were -0.163, -0.136, respectively (P<0.01). In women, Pearson correlation coefficients between age and hemoglobin, and hematocrit were 0.180 (P<0.05), 0.031 (P>0.05) respectively. Logistic regression analysis adjusted for age showed the lower odds ratio for anemia in both men and women without H. pylori than in those with H. pylori, but it was not significant.

Conclusion : H. pylori infection seems not to affect hemaglobin and hematocrit in the middled-aged men and the postmenopausal women with gastritis. Further investigations will be necessary for various individuals including infants and premenopausal women who may have high incidence for anemia.

Difference in hemoglobin between smokers and non-smokers.

Background
: The hemoglobin is a basic tool in diagnosing anemia. Not many studies compared hemoglobin between non-smokers and smokers in Korea and they did not differentiate hemoglobin between males and females. This study was conducted to show the difference of hemoglobin between non-smokers and smokers in male population and factors affecting hemoglobin.

Methods : We gathered 313 men who underwent periodic health examination from May to July 2001 in a general hospital. The relationship between cigarette smoking and hemoglobin concentration was examined by comparing the means of hemoglobin between two groups and through multiple regression analysis.

Results : The mean of hemoglobin was significantly higher in smokers than in non-smokers, especially in smokers with more than 1 pack per day. The factors that influence the hemoglobin level were age, the amount per day, and the total amount they smoked in the past(pack-year). Daily cigarette smoking seemed to cause a general upward shift of hemoglobin distribution curve, while age and pack-year on the opposite.

Conclusion : The study suggests that hemoglobin cutoff values should be adjusted for smokers and one must consider the patient's age to compensate for masking effect of smoking and aging on detection of anemia.
