Physical Activity Readiness Questionnaires for Post-COVID-19 Athletes: A Valid, Reliable, Low-Athlete Burden, Self-Assessment Screening Tool for Safe Return to Physical Activity and Exercise for Categories 1 and 2 COVID-19 Survivors

Background
No disease-specific, reliable, and valid self-assessment tools exist for athletes with mild coronavirus disease (COVID-19) to self-return to sports. This study aimed to develop a self-assessment screening method for athletes with mild COVID-19.
Methods
Item generation, item reduction, and pretesting were used to develop a questionnaire that served as a screening tool to detect severe post-COVID-19 complications. The questionnaire was validated by both internal and external reviewers, and the final version consisted of 11 items. Face validity was assessed through direct interviews with 11 athletes. Severe COVID-19 complications were identified for criterion validity and correlation analysis. A total score of 11 indicates that there is no risk of severe COVID-19 complications and gradual return to play is recommended; results <11 indicate a need for further evaluation by trained medical personnel. For reliability test, 50 respondents were chosen and asked to answer the questionnaires 7 days after their first responses.
Results
Physical Activity Readiness Questionnaires for post-COVID-19 athletes (PAR-COVID-Q) demonstrated excellent internal consistency (r=1, P<0.05) and test-retest reliability (r=1, P<0.05). A total of 118 athletes scored 11, had normal health screening, normal electrocardiogram and chest X-ray, and successfully returned to sports without any reported complications (r=1, P<0.05). Two athletes scored <11 and underwent further medical evaluation.
Conclusion
The PAR-COVID-Q is a valid and reliable self-assessment tool with a low burden for athletes to return to sports following mild COVID-19.

Silent Left Large Atrial Myxoma: A Patient with Serial Electrocardiogram Variation

Cardiac myxoma is often discovered as an incidental finding and presents with various subtle symptoms and signs. Electrocardiographic findings are mostly non-specific. Atrial flutter or conduction abnormalities are known to be rare. We report a case of large left atrial myxoma that was diagnosed by transthoracic echocardiography at a primary care clinic. An asymptomatic, 71-year-old woman presented with serial abnormal electrocardiogram changes during a routine consultation. A diagnosis of left atrial myxoma was obtained through transthoracic echocardiography. We report this case with a review of literature on cardiac myxoma associated with arrhythmia.

Association of Heart Rate Variability with the Framingham Risk Score in Healthy Adults

Background

The aim of this study was to investigate the relationship between heart rate variability (HRV), the Framingham risk score (FRS), and the 10-year risk of coronary heart disease (CHD) development among Korean adults.

Methods

The subjects were 85 healthy Korean adults recruited from a health check-up center. The FRS and 10-year risk of CHD development were calculated.

Results

The FRS in men was inversely correlated with the standard deviation of all normal to normal RR-intervals (SDNN); the root mean square successive difference (RMSSD); the percentage of successive normal cardiac inter-beat intervals greater than 20 ms, 30 ms, and 50 ms (pNN20, pNN30, pNN50); the low frequency (LF); and the high frequency (HF) (P < 0.05). There was no significant relationship between the FRS and HRV in women. Overall, in the receiver operating characteristic (ROC) analysis, the RMSSD, HF, SDNN, LF, LF/HF ratio, and pNN30 predicted an increased 10-year CHD risk. After adjusting for sex and body mass index, those with greater than one standard deviation in the RMSSD, HF, and LF had a 52-59% reduction in their 10-year risk of CHD development ≥ 10%.

Conclusion

This study therefore indicates that the HRV indices, particularly SDNN, RMSSD, pNN30, LF, and HF may be useful parameters for the assessment of CHD risk. Most notably, the usefulness of these HRV measures as indicators for CHD risk evaluation may be greater among men than among women.

Differences in Electrocardiography Findings Between the Healthy Elderly and Young Adults.

Background
: The elderly is known to have physiologic electrocardiographic changes with aging. But in Korea, such study is rare.

Methods : We investigated the residents in a community about past history of disease and current medications. Also we checked electrocardiography for those who were thought to be healthy.

Results : The elderly(≥60 year old) were 66 persons, and the young adults(<40 year old) were 8 persons. Mean QRS axis in the elderly was 37.7°, and it was significantly deviated to the left compared with that(63.8°) in the young adults(P<0.05). Mean T amplitude of V6 in the elderly was 3.47mm, and it was to some degree shorter than that(4.81mm) in the young adults(P=0.063). But pulse rate, PR interval, QRS duration, amplitude of R1, R6, S1 were not different between the elderly and the young adults.

Conclusion : Mean QRS axis in the elderly was significantly deviated to the left compared with in the young adults. And the mean T amplitude of V6 in the elderly was to some degree shorter than in the young adults.
