| Abstract |
Background
: Although there are many papers about clinical contents in primary health care of family medicine according to ICPC coding system, they are mainly limited to the metropolitan areas. So, this paper is designed to prepare data for residency training, and provide references to family physician who are to serve for primary health care of community prospectively by analizing clinical contents of new patients of a local family practice clinic in JeonJu, according to ICPC coding system.
Methods : Researcher visit a private family practice clinic operated by a doctor who had finished the course of residency training for Family Medicine, and distribute 3,655 new patients evenly by month and by areas from Jan 1990 to Dec 1990. Then sample randomly 1,000 patients and analyze their medical charts.
Results : In sex ratio of the subjects of investigation, female is superior to male, by 366(36.6%) males to 634(63.4%) females in 1,000 persons totally. And by age, first decade (0~9 years old) is the most common age group. The kind of Reasons For Encounters was 102 and cough is most common Reasons For Encounters by 272 cases(16.1%). The 91 kinds of diagnoses were used and Upper Respiratiory Infection accounts for the major portion by 195 cases(16.3%). A total of 1,352 diagnostic tests are performed when new patients are diagnosed- it is 1.35 tests for each person-, Complets Blood count is the single most test by 219 cases(16.2%). Referrals to other Department are 29 cases(2.9%) and General Surgery Department takes major portion by 10 cases(34.5%).
Conclusion : This paper is designed to prepare data for residency training and provide references to Family Physician who will serve for primary health care of community prospectively by analizing clinical contents of a local family practice clinic, according to ICPC coding system. |
| 초록 |
연구배경 : 가정의학의 일차 진료 내용에 대한 ICPC 분류법을 이용한 분석논문들이 많이 나와 있지만 주로 수도권 지역에 국한되어 있다. 본 연구는 지방도시에서 개원한 한 가정의학과 의원에서 진료한 초진 환자의 진료내용을 ICPC 분류법을 이용하여 분석함으로써 전공의 교육에 필요한 자료를 마련하고 향후 지역사회에서 일차 진료를 담당할 가정의들에게 참고자료를 제공하고자 하였다.
방법 : 가정의학과 전공의 과정을 수료한 개원 가정의의 진료소를 방문하여 1990년 1월1일부터 1990년 12월31일까지 1년간 진료한 초진환자 총 3,655명 중 월별, 지역별분포를 고르게 하여 그중에서 1,000명을 임의추출하여 이들의 의무기록지를 분석하였다.
결과 : 연구대상 환자 1,000명의 성별비는 남자가 366명(36.6%) 여자가 634명(63.4%)으로 여자가 많았고 연령군은 0~9세 까지의 연령군이 260명(26.0%)으로 가장 많았다. 내원 이유로는 총 102 종류로 분류되었으며 기침이 272예(16.1%)로 가장 많았고 진단명은 총 91종류로 분류되었으며 상기도 감염이 195예(16.3%)로 가장 많은 비율을 차지했다. 환자진료시 사용된 진단적 검사는 총 1,352건으로 1인당 평균 1.35건의 검사가 시행되었으며 말초혈액검사가 219예(16.2%)로 가장 많았다. 타과로의 의뢰는 총 29예(2.9%)였으며 일반외과가 10예(34.5%)로 의뢰율이 가장 높은 것으로 나타났다.
결론 : ICPC 분류법을 이용하여 지방도시 개원 가정의의 진료내용을 분석함으로써 전공의 교육에 필요한 자료를 마련하고 향후 지역사외에서 일차진료를 담당할 가정의들에게 참고자료를 제공하고자 하였다. |
