| Abstract |
Background
: Over the part 10 years, the family practice residency program has increased in numbers rapidly. But adequate evaluation of the program has not existed. Among other considerations for evaluating a residency program, the views of the residents themselves can help measure the nature of the residency program. To measure training satisfaction, the authors translated and modified a PSS (practice satisfaction scale), one of the scales that have been developed by previous study for the purpose of measuring physician satisfaction in various family practice residency programs.
Methods : This survey performed during the period from September to October 1992. About 310 family practice residents who had undergone residency training at the hospital in all of the provinces of the nation were study subjects. During the study period, the questionnaire asking about the type of residency program and the PSS of 12 items was mailed, and 160 were returned completed.
Results : The mean total satisfaction score of the subjects was 62.3±16.6, and the university-based residency programs are much more likely to satisfy the residents than the community-based programs. The influencing factors for PSS were type of training hospital and internal rotation rate. In general, the degree of satisfaction increased with increasing degree of total internal rotation rate and with decreasing degree of surgical part internal rotation rate.
Conclusion : In a family practice residency training program, the university based hospital with a higher total internal rotation rate and lower surgical part internal rotation rate was the most satisfying type of residency program perceived by residents. |

연구배경 : 지난 10여년동안 급속히 가정의학이 도입, 정착되어 오는 과정에서 가정의학 전공의 수련병원이 매년 늘어나고 있으나 이들에 대한 적절한 평가는 거의 이루어지지 않고 있다. 전공의 교육에 관한 여러 각도의 연구들 가운데 직접 현장에서 수련에 임하고 있는 전공의들로 하여금 수련에 대해 자체 평가하게 함으로써 유용한 정보들을 얻을 수 있을 것이다. 이에 저자들을 전공의 교육과정의 적절성을 평가하는 척도들 가운데 하나인 수련 만족도(Practice satisfaction scale)를 이용하여 여러 수련형태에 따른 수련만족도의 차이를 비교하고자 하였다.
방법 : 1991년 9월 1일 현재 전국의 가정의학과 수련병원에서 전공의 수련을 받고 있는 가정의학과 전공의 160명을 대상으로 1991년 9월 1일부터 10월 15일까지 미리 고안된 설문지를 이용하여 수련형태 및 수련 만족도를 조사하였다.
결과 : 조사 대상자의 평균 수련 만족도는 120점 만점에 62.3±16.6이었으며 대학병원 소속 전공의들이 지역병원 소속 전공의들보다 전항목에 있어서 통계학적으로 유의있게 만족도가 높았다. 수련만족도에 영향을 미치는 요인들 가운데 소속병원, 전체 수련중에서 내부수련이 차지하는 비율, 외과계 수련중에서 내부수련이 차지하는 비율 등이 통계적으로 유의하였다. 즉, 대학병원이 지역 병원보다, 전체수련중 내부수련의 비율이 높을수록, 외과계 수련중 내부수련의 비율이 낮을수록 만족도가 높았다.
결론 : 가정의학과 전공의 수련에 있어서 대학병원이면서 내부수련의 비율이 높고 외과계 수련은 외부파견 수련을 하는 형태가 전공의들에게 만족스럽게 평가되는 수련형태였다. |
