Effect of physician's antismoking education on patient's amoking behavior.

Abstract
Background
: The causative effect of cigarette smoking on pulmonary diseases, coronary heart diseases and much variety of cancers is evident. But the rate of cigarette smoking in Korea is increasing and adequate smoking cessation intervention has not been done yet. this clinical trial evaluated the effect of antismoking intervention done by primary care physicians.

Methods : The subjects were 145 smokers who visited SNUH Family Medicine Model Clinic from April to early July, 1991. We randomized subjects by their chart number. We educated all smokers about the adverse effect of cigarette smoking with self-help material and recommended to quit smoking. We reeducated the intervention group every 3rd, 5th and 7th week by face-to-face interview or telephone call. Twelve weeks after the intervention, we attempted to contact every smoker for the number of cigarettes per day currently smoked.

Results : The rates of smoking cessation and reduction for control group and intervention group were 7.7% and 38.5% vs. 19.7% and 47.99%, respectively. They were statistically significant(P<0.05). Statistically significant differences between the case and control groups were in their change of smoking habit in the following subgroups ; greater than 40 years of age, greater than 12 years of education and having diseases associated to smoking.

Conclusion : In conclusion, personal intervention carried out by primary care physicians resulted in significant smoking reduction and cessation. history.
초록
연구배경 : 흡연이 호흡기질환과 관상동맥질화 및 각종 암의 이환과 깊은 관련이 있다는 것은 널리 알려진 사실임에도 불구하고 우리나라에서는 흡연율이 계속 증가하고 있으며, 적절한 금연 프로그램이 없는 실정이다. 이에 본 연구에서는 외래환자를 대상으로 일차진료의사가 시행하는 금연교육의 효과를 분석하고자 시행하였다.

방법 : 1991년 4월1일부터 7월10일까지 서울대학교병원 가정의학과 외래를 방문한 흡연자중 의무기록번호의 끝자리가 짝수인 사람들은 대조군으로, 홀수인 사람은 대상군으로 정하였다. 처음 방문할 때에 모든 군에게 담배의 해독에 대한 교육자료를 이용하여 금연을 권고하였으며, 대상군에게는 금연날짜를 정하도록 권고하고 외래 방문 3, 5, 7주에 직접 또는 전화면담을 통해 금연을 재차 권고하였다. 첫방문으로부터 12주 되는 날에 금연여부와 그 양을 확인하였다.

결과 : 지속적인 금연교육을 시행한 대상군에서는 금연율이 19.7%, 흡연량의 감소는 47.9%였으며, 한번 금연권고를 시행한 대조군의 7.7%, 38.5%에 비해 통계적으로 의미있는 차이를 보였다(P<0.05). 흡연자의 특성별로 금연교육의 효과를 비교하면 40세 이상의 흡연자, 12년이상의 교육을 받은 고학력자, 흡연이 질병의 예후와 관련이 있는 질병을 가진 사람들에서 대조군과 대상군 사이에 통계적으로 의미있는 차이를 보였다.

결론 : 일차진료의사가 시행하는 금연교육은 금연율이나 흡연량을 의미있게 감소시켰다.
