Clinical study on necrotizing lymphadenitis.

Abstract
Background
: Determining the etiology of enlargement of the cervical lymph nodes is a common diagnostic challenge for clinicians. Necortizing lymphadenopathy was first described by Kikuchi in 1972, as a benign disease of which cervical lymphadenopathy with or without fever is characteristic. It has, on occasion, been misdiagnosed as tuberculosis lymphadenopathy or malignant lymphoma etc. It is resolved spontaneously without specific treatment within 1 to 4 months. So, we should consider this disease at the stage of presumptive diagnosis of the patient with cervical lymphadenopathy to avoid the unnecessary treatment of laboratory tests. Recently, in Korea, the cases with necrotizing lymphadenitis have been reported and studied, but the entity of this disease is not known to most of physicians of primary care. Therefore, we analyzed the clinical features of 39 cases which were diagnosed as necrotizing lymphadenitis by pathologist and reviewed the pertinent literature.

Methods : Thirty nine cases diagnosed as necrotizing lymphadenitis or subacute necrotizing lymphadenitis were selected from the files of the Department of Pathology, Kyung Hee University Medical Center, from 1985 through 1991.

Results : The age of 39 patients ranged form 5 to 58 years old with 89.7% below the 4th decades. Females were slightly more frequently encountered than males in the ratio of 22:17. The most common chief manifestations were neck mass only in 20 patients(51.3%). Cervical lymphadenopathy was multiple in 23 patients(60.5%) and tender in one half. The duration of the symptoms ranged from 1 week to 4 years and was within 3 months in the most of cases. At the first visit, the presumptive diagnosis were tuberculosis lymphadenitis in 23(59.0%), fever unknown origin in 6(15.4%), nonspecific lymphadenitis in 3(7.7%), malignant lymphomas in 3(7.7%), systemic lupus erythematosus in 1(2.6%), typhoid fever in 1(2.6%), lipoma in 1(2.6%), acute appendicitis 1 patient(2.6%). Three patients were relapsed.

Conclusion : At the diagnosis of cervical lymphadenopathy, the clinical and epidemiologic features of the enlarged lymph nodes are often useful in determining the diagnostic tests that are required and the type of initial therapy to be administered, also pathologic examination of excisional lymph node should be performed to prevent patients who have necrotizing lymphadenitis from unnecessary treatment or laboratory tests.
초록
연구배경 : 경부 림프절 종대는 임상에서 흔히 접하는 질환으로서 그 감별진단이 매우 중요하다. 1972년 Kikuchi에 의하여 처음 보고된 괴사성 림프절염은 경부림프절 종대를 특징으로 하고 다수에서 발열을 동반함으로써 흔히 결핵성 림프절염이나 악성 림프종을 의심하게 하는 질환이다. 이 질환은 거의 대부분의 환자에게서 1~4개월 내에 자연 치유되기 때문에 오진으로 인한 불필요한 치료를 피하고, 불명열로 진단되어 필요 이상의 검사를 하지 않도록 진단초기에 반드시 염두에 두어야 하는 질환이다. 최근에 우리나라에서도 이 질환에 대한 증례보고가 계속되고 있으나 아직도 본 질환에 대한 개념이 일반화되어 있지 못한 실정이다. 이에 저자등은 경부 림프절 종대의 한 원인인 괴사성 림프절염 39예의 임상적 특징을 연구하였다.

방법 : 1985년부터 1991년까지 7년 동안 경부 종괴 또는 발열 등의 증세로 경희의료원에 내원하여 조직검사상 괴사성 림프절염으로 진단된 39명의 환자를 대상으로 환자들의 의무기록을 조사하여 임상적 특성을 분석, 검토하였다.

결과 : 환자의 연령 분포는 5세에서 58세까지 였으며 30대 이하가 35명(89.7%)으로 대부분을 차지하였다. 남녀 비율은 여자가 22명(56.4%), 남자가 17명(43.6%)으로서 여자에서 다소 높은 빈도를 보였다. 처음 내원시 환자의 주증상은 경부종괴 만을 호소한 경우가 20명(51.3%)으로 가장 많았다. 경부 림프절 종대는 고립성이 15명(39.5%), 다발성이 23명(60.5%)이었고 압통을 동반한 경우는 19명(50%)이었다. 증상의 기간은 1주에서 4년까지 였으며 3개월 이내가 33명(84.6%)으로 대부분을 차지하였다. 첫 내원 시 임상적인 추정 진단은 결핵성 림프절염 23명(59.0%), 불명열 6명(15.4%), 비특이성 림프절염 3명(7.7%), 악성 림프종이 3명(7.7%), 전신성 흥반성 루푸스 1명(2.6%), 장티푸스 1명(2.6%), 지방종 1명(2.6%), 급성 충수돌기염 1명(2.6%)이었다. 3명(7.7%)은 과거에 괴사성 림프절염을 진단받은 재발된 경우였다.

결론 : 경부 림프절 종대 환자의 감별진단을 위해 진단 초기에 림프절 조직 생검을 시행하는 것이 바람직하다. 특히 불명열의 감별진단에서 반드시 괴사성 림프절염을 염두에 두고 세밀한 신체검진 조직 생검을 시행하는 것이 조기에 진단하고 불필요한 치료를 피할 수 있는 길이다. 향후 경부 림프절 종대를 가진 모든 환자들을 대상으로 연구를 확대하여, 전체 림프절 종대 환자중 괴사성 림프절염의 빈도를 파악하는 일과 이 질환의 임상적 연구가 더 진행되어야 할 것으로 판단된다.
