C-reactive Protein, Obesity, and Cardiovascular Risk Factors in 7 Years-old Korean Children.

Abstract
Background
C-reactive protein (CRP) is a nonspecific, but sensitive marker of systemic immune response. Many studies have suggested that CRP levels are associated with obesity and cardiovascular risk factors not only in adults, but also in children. We assessed the inter-relationships between CRP, obesity, and cardiovascular risk factors in Korean children aged 7 years old. Methods: A total of 126 children (40 overweight and 86 normal weight children) were recruited from seven elementary schools in Seoul and Gyeonggi-do. Physical examinations including various obesity parameters were done. Fasting and post-prandial venous blood was withdrawn for the chemistry. The hsCRP was measured by the Nephelometry. Results: The hsCRP was associated with body weight, body fat (%), body mass index (BMI), waist circumferences, hip circumferences and waist/hip ratio (P<0.05). From the lowest to the highest quartile groups of hsCRP, mean fasting insulin levels (2.7⁑3.1μIU/mL, 2.6⁑2.1μIU/mL, 3.4⁑3.7μIU/mL, and 4.2⁑3.6μIU/mL, respectively)(P<0.05) and HOMA-IR levels (0.60⁑0.70, 0.59⁑0.49, 0.74⁑0.84, and 0.95⁑0.83, respectively)(P<0.05) increased. Serum lipid profiles were not associated with the hsCRP levels. Conclusion: In children aged 7 years old, the hsCRP was significantly associated with obesity and cardiovascular risk factors. 초록
연구 배경: C-반응성 단백(C reactivie protein, CRP)은 비특이적인 급성기 반응물질로서 전신적 면역반응의 민감한 지표이다. 최근에는 건강한 성인에서 CRP와 비만 및 심혈관계 질환 발생과의 연관성이 보고되고 있으며, 서구의 연구에서는 소아에서도 비만도가 높을수록 CRP가 높은 것으로 나타나고 있다. 본 연구에서는 우리나라 초등학교 1학년인 7세 소아에서 비만도와 CRP 농도의 연관성에 대하여 알아보고, CRP 농도가 심혈관계 질환의 위험 인자와 관련되어 있는지 알아보고자 하였다. 방법: 서울 및 경기도 지역의 7개 초등학교에서 자원한 1학년 126명(과체중아 40명, 정상체중아 86명)을 대상으로 비만도 측정을 포함한 신체계측, 혈압 측정 및 혈액 검사를 실시하였다. High sensitive CRP (hsCRP)는 Latex 응집비탁법을 이용하여 측정하였다. 상관분석을 사용하여 hsCRP와 비만도 및 심혈관계 위험인자와의 관련성을 알아보았다. 결과: hsCRP의 농도가 높을수록 체중, 체지방률, 체질량지수, 체질량지수 백분위수, 허리둘레, 엉덩이둘레, 허리/엉덩이 둘레비가 증가하였다(모든 P<0.05). 심혈관계 위험인자 중 식전 인슐린과 인슐린 저항성을 나타내는 HOMA-IR이 hsCRP와 유의한 관련성을 보였다. 식전 인슐린은 hsCRP 사분위수군에서 각각 2.7⁑3.1 μU/mL, 2.6⁑2.1 μU/mL, 3.4⁑3.7 μU/mL, 4.2⁑3.6 μU/mL으로 유의하게 증가하는 경향을 보였으며(P<0.05), HOMA-IR도 각각 0.60⁑0.70, 0.59⁑0.49, 0.74⁑0.84, 0.95⁑0.83으로 유의하게 증가하였다(P<0.05). 이외 수축기 이완기 혈압과 혈청 지질치는 hsCRP와 유의한 관련성을 보이지 않았다. 결론: 초등학교 1학년 소아에서 hsCRP는 비만도 및 심혈관계 위험인자와 유의한 관련성을 보였다.
