상부위장관 출혈의 임상적 고찰 (A Clinical Observation of Upper Gastrointestinal Bleeding)

Abstract

This study is a review of 75 cases of upper gastrointestinal bleeding admitted in Bucheon Daesung Hospital during the 5years period from May 1, 1984 to April 30, 1989.
The following results were obtained ;
1) The male-female ratio was 8.4 : 1 and peak incidence was in the 5th decade(26%)
2) The causes of bleeding were ; peptic ulcer in 48%, hemorrhagic gastritis in 17%, esophageal varices in 13%.
3) The seasonal distribution was as follows ; summer(29%), winter(26%), spring(24%), autumn(21%).
4) The modes of upper gastrointestinal bleedeng consist of hematemesis in 39%, melena in 20% and the combination of hematemesis and melena in 41% of the cases.
5) The hemoglobin value on admission showed above the 10gm% in 48%, 7-10gm% in 36% and below the 7gm% in 16% of the cases.
6) The amount of transfusion was as follows ; below 4 pints in 52%, above 9 pints in 24%, and 5-8pints in 8%.
7) In the management of bleeding, 89% of cases were treated conservatively, 11% surgically.
8) The overall mortality rate was 12%, The mortarlity rate with conservative treatment was 12% and that of surgical treatment was 12.5%.
