The relationship of common reasons for encounter and major diagnoses of patients admitted to a hospital.

Abstract
Background
: It is important to know the contents of health problems in patients in a primary care setting. The aim of this study was to explore the main chief complaints and major diagnoses of patients, who were admitted to a hospital using the ICD-10 and to observe difference according to each department and admission route and how diagnoses were made.

Methods : A total of 18,560 patients who were admitted to a hospital located in Chungnam Province from 1 Jan 1998 to 31 Dec 1998. Mean cheif complaints and major diagnoses made through the admissions departments were chosen as subjects (medical vs. surgical) and by admission route (emergency vc. OPD) were analyzed. How the diagnoses were derived from the most common chief complaints among medical and surgical departments were analyzed by admissions departments and by admission route.

Results : The most common 10 chief complaints revealed no significant difference by admission route in medical departments, but there was some difference in surgical departments. There was some difference in the most common 10 major diagnoses by admission routes in both medical and surgical departments.
Abdominal and pelvic pain, which was the most common chief complaints, became a significantly different diagnosis by admission route in both departments.

Conclusion : Main chief complaints were similar regardless of admission routes, but the diagnoses were different. ICD-10 classification may be useful to classify the diagnoses, but have limitations to classify chief complaints or reasons for encounter. It is necessary to introduce a new classification such as ICPC-2 for reasons for encounter in order to explore the dimension of health problems.
초록
연구배경 : 일차의료의 특성 중에서 진료의 포괄성은 환자의 건강상의 문제를 대부분 해결할 수 있어야 한다는 것을 의미하며 환자의 건강문제를 파악하고 그 크기를 측정하는 것이 필요하다. 환자의 건강문제의 종류와 크기를 측정하기 위하여 입원환자의 입원시 주호소 증상과 퇴원시 진단이 입원 경로와 입원 과에 따라 다른 분포를 보이는지 조사하였다.

방법 : 1998년 1월 1일부터 12월 31일까지 1년 동안 1개 대학병원의 퇴원환자 전수 중 의무기록이 있는 18,560명을 대상으로 하였다. 제 10차 개정 국제질병분류(ICD=10) 체계를 이용하여 분류한 환자의 주호소증상(chief complaints)과 퇴원시 주요 진단명을 입원과별(내과계, 외과계) 및 입원 경로별(응급실, 외래)로 비교하였다. 또 내과계와 외과계에서 모두 가장 흔한 주호소 증상이 퇴원시에 어떤 진단명으로 판명되는지 분석하고, 입원경로에 따른 차이를 비교하였다.

결과 : 내과계 환자의 흔한 주호소 증상은 입원경로에 무관하게 복부 및 골반 동통이 가장 많았고, 상위 10위에 포함된 주호소 증상에는 차이가 없었다. 외과계 환자의 주호소 증상은 입원 경로와 무관하게 복부 및 골반 동통이 가장 많았으나 1위 외에 10위 내에 포함된 주호소 증상은 입원 경로별로 차이가 있었다. 퇴원시 주요 진단명은 내과계와 외과계 모두 입원 경로에 따라 차이를 보였다.
가장 흔한 주호소 증상이었던 복부 및 골반 동통 환자들의 퇴원 시 주진단명을 비교한 결과 입원 경로에 따라 서로 다른 진단으로 분류되었다.

결론 : 입원경로에 무관하게 주호소 증상은 비슷한 반면 퇴원 시 진단명은 차이를 보였다. ICD 10 분류는 최종 진단명을 분류하는데 유용하나, 지나치게 다양한 진단명을 포괄하여 주호소 증상을 분류하는데는 한계가 있는 것으로 생각된다. 건강문제의 부담의 크기를 파악하기 위해 ICPC 2 분류와 같이 ‘진료의 에피소드’를 기준으로 하는 새로운 분류법의 도입을 고려하는 것이 필요하다.
