Smith Hearing Screening ( SHS ) 를 이용한 청력장애 선별검사 (Original Articles : Smith Hearing Screening Test for Identifying Hearing-Impaired Patients)

Abstract
Background
: Hearing loss is one of the most common of all physical impairments, but physicians seldom screen adults for it, and patients often overlook hearing problems. The development of screening instruments to detect hearing impairment in populations is an area of active research. A new hearing impairment screening questionnaire, the Smith Hearing Screening(SHS), was developed by Mary F. Smith, PhD which was made up of total 14-items ascertaining hearing impairment-related information. This study was conducted to estimate validity and reliability of SHS questionnaire using portable audioscope(Welch Allyn Inc.).

Methods : Self-administered screening questionnaire(Smith Hearing Screening) and a hearing screening evaluation using the Welch Allyn Audioscope instrument were given to 176 consecutive family practice patients over the age of 18 years during the period from June 1993 to August 1993. By estimating sensitivity, specificity, positive predictive value, negative predictive value of Smith Hearing Screening(SHS) questionnaire, validity of SHS was assessed. And reliability of SHS was assessed by 1 month test-retest method.

Results : The prevalence of hearing impairment in this sample was 15.34%(27patients). Starting with Smith recommended cut-off score of 6 or greater, the sensitivity of SHS was 70.37% and specificity was 86.58%. Starting with cut-off score of 5 or greater, the sensitivity of SHS was 77.78% and specificity was 77.85%. SHS's reliability measured by test-retest correlation coefficient in 30 patients by random telephone follow-up interviews, who had reported that there was no change of SHS score between test(Time 1) and retest(Time 2) period, was 0.923(P<0.001).

Conclusion : The SHS questionnaire is valid, reliable, easy and inexpensive tool for detecting hearing impairment in adults. Since hearing impairment is a very common problem throughout the world, authors recommend that all adult patients be evaluated for hearing impairment.
초록
연구배경 : 청력장애는 신체장애 중에서 가장 흔한 것 중의 하나이나 난청환자의 조기발견 및 치료는 거의 등한시 되어 온 것이 사실이다. 1992년 Smith가 새로이 발표한 총 14문항으로 된 설문선별검사 방법이 있어 그 유용성을 살펴봄으로써 청력장애자의 조기 발견 및 청력개선에 대한 기초자료로 삼고자 본 연구를 시행하였다.

방법 : 1993년 6월부터 8월까지 서울 소재 대학병원 가정의학과 외래를 방문하여 검사에 동의한 18세 이상의 성인을 대상으로 Smith Hearing Screening(SHS) 설문조사후 Welch Allyn Audioscope을 이용하여 순음청력검사를 시행하였다. SHS설문조사의 민감도, 특이도, 양성예측도, 음성예측도를 측정하여 설문지의 타당도를 살펴보았으며 1개월뒤 전화방문하여 SHS설문을 재조사한 후 각 항목의 신뢰도를 알아보았다.

결과 : 전체조사 대상자 176명중 청력장애자는 27명(15.34%), 정상인은 149명(84.66%)이었으며 남자가 86명, 여자가 90명이었다. SHS설문에서는 Smith가 제시한 6점을 기준으로 하였을 경우 민감도가 70.37%, 특이도는 86.58%로 나타났으며 기준점을 5점으로 하였을 때는 민감도가 77.78%, 특이도는 77.85%로 나타났다. 설문지의 각 항목의 신뢰도는 재검사법에 의한 상관계수(test-retest correlation coefficient)로 표시하였으며 설문지(total score)의 재검사 신뢰도(r상수)는 0.923이었다(P<0.001).

결론 : 청력장애에 대한 선별검사를 위해 만들어진 Smith Hearing Screening(SHS) 방법은 외래 환자진료시 사용하기가 간편하며 타당도 및 신뢰도가 높은 유용한 선별검사 방법으로 여겨진다. 또한 청력장애는 신체장애중 가장 흔한 것 중의 하나이며 본 연구에서도 청력장애에 대한 유병률이 15.34%로 높게 나타났다. 따라서 일차진료를 담당하고 있는 가정의는 모든 성인에서 청력장애에 대한 선별검사를 시행해야 할 것으로 사료된다.
