Other intestinal diseases in operation cases under preoperative diagnosis of acute appendicitis.

Abstract
Background
: Acute appendicitis is a common disease which needs surgical treatment among acute abdomen, but the specific sutdy is applied very seldom because of emergency operation neccessity. Therefore this study was carried out to find out a way of help for preoperative diagnosis and treatment of this diseases, to analyze intestinal diseases except appendicitis among operation cases of appendicitis preoperatively.

Methods : For 3 years from August 1990 to July 1993, we reviewed clinical reports which 25 cases of intestinal diseases among 881 cases of performed operation under preoperative diagnosis of acute appendicitis, and analyzed sex and age distribution, complete blood count, disease distribution, postoperative diagnosis, operative method, pathologic diagnosis and postoperative progress.

Results : The age distribution shows that 4th decade is the highest incidence, and male and female ratio is about 5 to 1. We cannot find any differences between these intestinal diseases and appendicitis in symptom, physical examination and leukocyte count. The mean duration of symptom was 6.1 day that is longer than acute appendicitis. Benign diseases were dominant, and cecal diverticulitis was the highest, and others such as cecal tuberculosis, cecal inflammatory mass, cecal cancer, Meckel's diverticulum, appendix carcinoid tumor, appendix mucocele and appendix cancer were found.

Conclusion : If the 4th decade male patient who were suggested acute appendicitis and duration of symptom is longer than that, detailed history taking with radiologic approach must be taken with a deep concern. It must be considered diverticulitis in 4th decade and malignant disease as well in over the 5th decade. When the opinion is different from clinical symptom before operation and findings in operation, more careful stances are essential for trying to find out other intestine with doubt.
초록
연구배경 : 급성 충수염은 급성 복증중 외과적 처치를 요하는 흔한 질환으로 감별해야할 질환들이 많이 있고, 수술전 임상 증상, 이학적 소견등으로 감별이 어려운 경우가 많으나 응급 수술을 요하는 질환이므로 수술전 특수검사를 시행하지 않는 경우가 대부분이다. 이에 충수염이 의심되어 수술한 예들중에서 충수염이외에 발견된 장관 질환들을 분석하여 이들 질환들의 수술전 진단 및 치료에 도움이 되고자 본 연구를 시행하게 되었다.

방법 : 1990년 8월부터 1993년 7월까지 만 3년동안 본원에 내원하여 급성 충수염의 진단하에 수술을 시행한 881예중 급성 충수염 이외에 발견된 장관 질환 25예를 대상으로 이들의 임상기록지를 조사하여 성별, 연령별 분포, 임상증상 및 기간, 이학적 소견, 말초 혈액 검사소견, 질병 분포 및 빈도, 수술 소견 및 수술방법, 병리 조직학적 소견과 수술후 경과 등을 분석하였다.

결과 : 연령 분포는 30대(36%)가 가장 많았고 성별 분포는 약 5:1로 남자가 많았다. 증상 및 이학적 소견, 백혈구의 수에서 급성 충수염과 감별할만한 차이점을 발견하지 못하였으나 증상의 평균 기간은 6.1일로 급성 충수염의 경우보다 길었다. 질병 분포상에서는 양성 질환(92%)이 우세했고 이들중 맹장 게실염(64%)이 가장 많았으며 그외에 맹장결핵, 맹장의 염증성 종괴, 맹장암, Meckel씨 게실, 충수 유암종, 충수 점액종, 충수암이 발견되었다. 수술시 충수 점액종과 맹장 종괴가 의심된 예들은 우측 대장 절제술을 시행하여, 병리 조직학상 양성 충수 점액종과 맹장 게실염, 맹장 결핵, 맹장의 염증성 종괴, 맹장암들로 진단되었고, 충수염이 의심된 예들 중 충수 절제술을 시행한후 병리조직학상 충수암과 충수 유암종으로 진단된 예가 있었다. 맹장 게실염과 Meckel씨 게실이 의심된 예에서는 상황에 따라서 수술을 시행하여 게실 절제술을 한 경우들은 병리 조직학상 모두 게실 질환으로 진단되었다. 수술시 맹장 게실염이 발견되었으나 충수 절제술만 한 2예에서 우하복부 동통과 혈변이 발생하였다.

결론 : 급성 충수염이 의심되는 30대 남자 환자에 있어서 급성 충수염에 비하여 증상의 기간이 긴 경우에 충수염 이외의 다른 장관 질환에 관심을 두고 문진시 자세한 병력 청취와 함께 방사선과 진단기술을 이용한 적극적인 진단적 접근이 필요하며, 30대와 40대의 환자에서는 게실염, 40대 이후의 환자에서는 악성 질환의 가능성을 염두에 두어야 할것이다. 또한 수술시에 게실염이 발견되면 절제하는 것이수술후 경과가 좋고 수술전의 임상 증상과 수술시의 충수염과의 소견이 맞지 않을때 의심을 가지고 충수이외의 다른 장관들을 관찰하는 자세가 필요하리라 생각된다.
