Body Mass Index Change during and after the Childhood Obesity Prevention Program.

Abstract
Background
This study was designed to find out the effect of school based obesity prevention intervention by considering the changes of BMI during both at the time of intervention and post-intervention. Methods: The research was conducted at an elementary school located in Busan metropolitan city, where we measured the height and weight of 260 students at baseline, 6 months after the initiation, and 6 months after the intervention. The students were classified into three weight groups of normal, overweight and obesity according to the age and sex-specific BMI. Results: The BMI decreased or maintained in 60% of the boys and 65% of the girls after the initial six months of the intervention, while after six months from the end of the intervention, BMI increased in 91% of the boys and 83% of the girls. The children's BMI significantly decreased during the intervention (P<0.01), whereas it significantly increased post intervention (P<0.05). In the logistic regression model including age, sex, initial BMI, and the follow-up status after the intervention, the increase of BMI by 1 unit was associated with 11% lower odds (95% CI 0.83∼0.98) for BMI decrease and maintenance. Likewise, the increase of age by 1 year was associated with 24% lower odds (95% CI 0.59∼0.98) for BMI decrease and maintenance. However, there was no significant predictors for BMI decrease and maintenance after the intervention. Conclusion: As the positive effect of this intervention for children is not consistent, the research should be more focused in finding out the effective method for prevention of obesity among elementary school children, as a better solution in the longrun. 초록
연구배경: 소아비만 예방을 위한 학교 프로그램이 시행기간 동안 및 종료 후에 아동들의 체질량지수에 변화를 파악함으로서 소아비만 예방 교육의 지속적 효과를 알아보았다. 방법: 부산광역시에 소재하는 1개 초등학교 학생들을 대상으로 2004년 3월에서 9월까지 학교에서 시행한 소아비만 예방 프로그램 및 프로그램이 종료된 6개월 후 체중과 신장을 측정할 수 있었던 260명(남아 127명, 여아 133명)이 본 연구의 대상이었다. 성별 연령별 체질량지수를 이용하여 정상체중, 과체중, 비만군(대한소아과학회, 1998)으로 분류하였다. 결과: 소아비만 예방 프로그램이 시행된 6개월 후 60%의 남아들과 65%의 여아들에서 체중이 줄거나 유지된 반면에 프로그램 종료 시점에서 6개월 후에는 91%의 남아들과 83%의 여아들에서 체중이 증가하였다. 프로그램 시행 전에 비하여 시행 6개월 후 체질량지수가 유의하게 감소하였으나(P<0.01) 프로그램 종료 시점에 비하여 종료 6개월 후에는 유의하게 증가하였다(P<0.05). 성별, 연령, 프로그램 시행 후 추적 유무, 초기 체질량지수를 예측변수로 포함한 모델에서 초기 체질량지수가 높거나(유병교차비 0.89, 95% 신뢰구간 0.83∼0.98) 연령이 많으면(유병교차비 0.76, 95% 신뢰구간 0.59∼0.98) 프로그램 시행기간 동안 체질량지수가 감소하거나 유지될 가능성이 낮았다. 그러나 비만 예방 프로그램이 종료된 6개월 후 아동의 체질량지수 감소 및 유지에 대하여 성별, 연령, 초기 체질량지수, 프로그램 기간동안 체질량 지수 변화량은 모두 유의한 예측 요인이 아니었다. 결론: 초등학교에서 시행한 소아비만 예방 프로그램이 아동들의 체질량지수에 미치는 긍정적 효과는 지속적이지 않았으므로 소아비만 예방에 장기적으로 효과적인 방법을 모색하여야 할 것이다.
